I want to adopt my two step-daughters.About ten years ago my wife was awarded sole
custody and because the circumstaces of their separation her ex was served a restraining
order and also ordered to pay child support but has never sent a payment. As such, wil it
still be necessary to send him a consent and waiver form?

Yes your wife will have to get ex's voluntary relinquishment of rights and his consent to the adoption.
