Adult Adoption

What is the name of your state? LA
I am 32 and from my understanding, I have to have my biological father adopt me back to have him as my legal father again. Do I have to have a lawyer to have this done? If not any advise on the procedure to do this? Do I have to have a legal name change also since I am over the age of 18?
