What is the name of your state? wisconsin when applying for a birth certificate for passport. do you put your biological parents or the parents that adopted you when you were a year old?

Well, I'm in WI, and my daughter was readopted in WI, and the only parents or birth certificate that would matter for passport would be her CURRENT birth certificate, showing her parents, not her biological parents.

Certification of Birth records for adoptees are obtained from Madison, regardless of your county.
