change name

my son was adoped by his step father (my husband) when he was 8 years old he is now 21 and is living with his biological father he would like to take his born name back how does he accomplish this task? we live in Louisiana he live in Nevada now

in most states you can have your name changed to whatever you want. he will have to call the court house and they will give him the information about how to go about changing his name. it will cost a fee usually around $20 or so bucks and he will be issued a new SS card from the SSA.
