What is the name of your state? Mine California. Daughter's Wisconsin

My daughters stepfather is adopting and I am voluntarily terminating my parental rights. They sent me 16 pages of forms to sign. Which one am I to sign? The forms are titled the following:
Petition for Termination of Parental Rights

Summons (Termination of Parental Rights)

Notice and Order of Hearing (For Publication)

Consent to Use Mother's Name for Publication

Uniform Child Custody Jurisdiction Act Affidavit

Consent to Termination of Parental Rights (Judicial)

Consent to Termination of Parental Rights (Affidavit)

Order Concerning Termination of Parental Rights (Voluntary)

Order Concerning Termination of Parental Rights (Involuntary)

I'm just really confused by all the forms. I need help and can't afford a lawyer. Can someone help me? It is really hard to get help for non-custodial fathers. I feel we are looked down upon despite others knowing the whole truth. Thanks for the help.
