The adult children want to do this. Is there an easy way to do this? A simple document to file at the courts? Do I need a lawyer or is this something I can do myself?

Contact an adoption attorney. It should be a fairly simple thing that would not be terribly costly.
However, there are other ways to deal with names and inheritances. Its a very simple matter for their names to be legally changed.....and a solid will takes care of any inheritance issues.
