need help

What is the name of your state (only U.S. law)? OHIO.

I need to know what form to file to contest an adoption? I was miss lead and my ex-wifes attorney lied on the paper work and I have prove of that. I know I have a year to contest. I just need to know what paper work to file what form I live in Fl but I need to file in Ohio.
