reverse an adoption

What is the name of your state? Louisiana
I was adopted by a step father when I was 9yrs. old. I no longer has contact with that step father and I have a wonderful relationship with my biological father. I know that it can be done but I don't know how to go about doing it but I want to have my adoption "reversed" (I know that is not the term for it but I can't remember what is). I just need some advise on how to start. I want to have my biological dad put back on my birth certificate and my last name changed back to by biological last name.

Any help is appriciated.

How old are you now?

If you are past the age of majority it becomes an adult adoption by your biological father. Others will come along and let you know how specific that can be and if it would terminate your mother's rights or not... I"m not sure in those cases. If you are past majority I do not believe your father needs to be notified. However if you are still a minor he would and would have the right to contest.

At any rate your biological father would have to adopt you.
