stepparent adoption

What is the name of your state (only U.S. law)? North Carolina- I have a daughter from a previous relationship whose father has not paid child support or had any contact with her in almost a year (not by my doing). I have been married for 5yrs, and she considers my husband to be her dad (calls him dad and her bio one by his first name). I want to terminate her fathers parental rights so that my husband can adopt her. She has talked about this and wants to be adopted. She is 9 and very intelligent and mature. How do I go about doing this when NC, TN and I cannot find my ex?

You hire an attorney.

Termination of parental rights and adoptions are not do it yourself projects for someone who is not legally savvy.

(and I won't even roll my eyes at that as you did)

There are NOT forms for that. Rolling your eyes is NOT a good way to get help.
