Utah Step Parent Adoption

What is the name of your state? Utah

I would like to know if anyone is aware of a site that would allow me to access all the necessary forms to complete a step parent adoption in the state of Utah.
I located my child's biological father and he is willing to sign off on everything. I do not expect it to be dramatic or complicated. My husband and I have been married for 5 years so I know we are past the required time frame on that.
I'm hoping to offset the cost of an attorney. Anyone know where I can download the forms?
