Its possible. Nursing home contracts are often structured so that the nursing home becomes the rep payee for social security and pensions, unless the person in question has sufficient assets to truly cover the nursing home bill, with a significant amount left over.
If there are sufficient assets to cover the nursing home bill with some left over, then the patient usually has a financial guardian or POA who would be responsible for paying the alimony payment, just like any other bill.
However, realistically your mother in law needs to be planning on how she is going to live her life without alimony....because the time is coming.
