bad check/failure to appear

What is the name of your state?PA
I issued a personal check to a local grocery store on Nov 28,2004. At the time I know that I had over $1000 in the account, the check was for 57.00. On the 1st of Dec, my account waws frozen due to a law suit against me. I was unaware that the account was frozen until about Dec. 5th. Many checks were returned unpaid. I paid all of the returned checks except for this one. I went to he store to pay this check, only to be told that they didn't accept payment for bad checks, but that a women downtown handled them. They gave me her phone number which I called on 3 different ocassions only to not get any answer. I then managed to get my account unfrozen around the first week in Jan. By this time I had no idea which checks were good or bad, the account was so screwed up by then. I assumed they had resubmitted the check and it had been paid. In July of 2005, I received a notice to appear within 10 days. The next day I had laser eye surgery due to diabetes, and 4 days later I had eye lid surgery due to diabeties. During all of this I simply forgot about the notice to appear. Yesterday I then received a notice for failure to appear witha warrent for my arrest. What do I do, go to the courthouse? Get an attorney? Will be arrested if I turn myself in, and placed in jail until court? I've never had any legal problem like this and don't know what to do. Please advise.
