This is a bench warrant you are talking about?

Generally what you do to quash a warrant is go to court and see the court clerk. At the clerk's office, you sign a request for a motion hearing to quash the active warrant. Then you sign a promise to appear. Then you appear.

There is no form or template.

edit to add: When you go to the clerk's office, be prepared to pay the warrant fee.

Depending on what you are wanted for, it is possible you will get to see the judge that day. Of course, you get to wait in jail till then.
