Accident on job

My insurance company told me I need a letter stating that I was working when I got in an accident with a company owned and insured vehicle from my ex-employer. I called and faxed them with no response. This letter they said is needed so I don`t get penalized with higher insurance rates. Is there anyway I can force my ex-employer to give me this letter or are there other ways to get the info the insurance company needs.
