i had an accident about 3 months ago with a 17 seventeen
year old guy, he was uninsured i had to pay for my rent a car
while my car was on the shop.
is there a form that i can fill out to put
a claim gainst this person to get my money back?

or anything i can do with out going to court?

There is no form.

The only thing you can do without going to court is to send him a certified return receipt letter asking for re-imbursement or your out-of-pocket expenses. Include photo copies of all the related receipts.

If he ignores you then you will have to go to the self help desk at the court house for a packet and information on filing in small claims court.
