disability qualifications

I have suffered from severe migraines for about 3 years now. I've been to several doctors, including a neurologist who specializes in migraine. I have only gotten worse. According to the MIDAS questionnaire, I have a severe disability (grade IV), and that's WITH treatment! How do I find out if I qualify for disability, and how do I apply? I live in Georgia.
