Duty of reporting crimes

What is the name of your state? Iowa

If you see an individual showing threatening behavior to him/herself or others and is illegal, are you required to report the incident to authorities? What if it is a situation where the individual does not seem aware of what he/she is doing or incapable of understanding his/her actions?
