What is the name of your state (only U.S. law)? Oklahoma


I was in a car accident approximately 18 months ago in which I was reared ended while stopped at a light. After going through a few doctor visits and rehab, I decided to settle with the insurance company. I submitted my bills to them and now the claims rep has sent me a "Medical Authorization Form" to allow this insurance company to review all my medical records.

I do not feel good about doing this. Is this a form I should sign and not worry about? Is this a standard type form used by all insurance companies?

Yes, it's standard. If you want them to pay, you have to sign the form.
