In Decenber 2000 while stopped at a red light I was rear ended by a lady on a cell phone. Car damages were just about $1000.00 My medical bills have just been $375.oo. Back a little sore from time to time. Her insurance paid for the body damages to the car. Before I submit my medical am I entitled to any other compensation. I would like to do this with out a lawyer. Is there a form letter or standard formual to ask for damages? When I submit my medical bill do I need to include the doctors written report?
