What is the name of your state? Massachusetts.

My wife was doing shopping at a supermarket in North Andover, MA when a cupboard from an apple juice display setup fell on her head while she grabbed a bottle of apple juice. Subsequently she suffered from headache and muscular back pain.

Questions:
1) Is the supermarket responsible for her injury (from their liability insurance?)?
2) Should my wife send the original doctor bill to the Supermarket for them to handle ?
3) Should my wife pay the doctor co-payment first and send the co-payment receipt to the Supermarket (in this case, the cost will be covered by our insurance)?
4) Any other compensation (e.g. suffer from pain, stress etc) can we seek from the supermarket?
5) Do we need to get a lawyer to seek compensation?
