Subrogation/Deduct Premuim Costs?

I live in MO. I think my health-plan contract allows for reducing the reimbursement amount by the cost of premiums and related attorney and court costs -- how does one go about calculating this .. and if your own medical payments is being subrogated, and the medical payments premium cost is part of the entire policy -- how much over how long can you deduct?
