Not really sure what goes into spots a, b, or c. Possibly my name into all three?
Also, what goes into "d" and "e" as an official title. Do I need a notary or something to witness this? Or can my roommates witness it, and what would they put in the title spot.
Any help would be greatly appreciated!

Okay, seriously, this part is called the "Jurat". It's for the use of the Notary Public. Go to your bank, and use their Notary services.
That's all. Then, have the bank make a photocopy for your records, and mail the original back to the insurance company.
