Translation please....


What is the name of your state? Oregon

I was hit by a driver a few months back and his insurance company is sending me a settlement after I sign a release and send it back to them.
Problem is, I don't understand how to even fill the bottom part of the form out!

Certificate of Witnesses

We certify that this release was signed in our presence by the above who acknowledge that he/they understood it fully.

County of______________
On this______day of______20___ before me personally appeared ____(a)_____ to me known to be the person(s) named in and who executed the above release and acknowledged that ____(b)_______ executed the same as_____(c)________own free act and deed.

(Official Title)

Not really sure what goes into spots a, b, or c. Possibly my name into all three?
Also, what goes into "d" and "e" as an official title. Do I need a notary or something to witness this? Or can my roommates witness it, and what would they put in the title spot.
Any help would be greatly appreciated!

Okay, seriously, this part is called the "Jurat". It's for the use of the Notary Public. Go to your bank, and use their Notary services.

That's all. Then, have the bank make a photocopy for your records, and mail the original back to the insurance company.

A = You
B = 1st witness
C = 2nd Witness
D = Notary
E = Notary Official Title

I could be wrong but that is what it looks like. You might want to have an attorney look over your settlement before signing.

Darn you IAAL *shakes fist*
