Agreement to Compromise Debt

What is the name of your state? Wisconsin
I have another question about the agreement from the credit info center. Do I list the collection agency and the original creditor under "referred to as Creditor" in the contract? Can the collection agency sign this agreement (contract)?

thanks again for all the help...I hoping to end this very soon.
