Proof of claim vs bankruptcy

I own shares in a company that has filed for Ch. 11 bankruptcy. I received a form to "file proof of claim." Does this filing apply to stockholders? How do I determine value of my stock or value of my claim? I reside in Calif; company is domiciled in Belgium (Lernout & Hauspie). When claims are processed, are stockholders the last in line to get assets?
