What is the name of your state (only U.S. law)? wv

Is a corporate who files for bankruptcy chapter 11 still liable to pay out vacation pay to its employees? If so do I have to fill out a claim form to be paid? If I fail to fill out the forms does it mean that I forfiet my rights to be paid vacation pay?
