What is the name of your state (only U.S. law)? MASS

I plan to buy a home, also have MA salesperson license associated with a small broker with little support. Do I need to use "Massachusetts Mandatory Licensee Consumer Relationship Disclosure" form when I send the Offer for Purchase?

if not, what is the appropriate form to inform? Thanks.

In some states, one sentence is allowable on the contract, EX: Buyer holds an active ( inactive) real estate salesperson's license in the state of MA.
