Form 34 - Addendum to P & S

What is the name of your state (only U.S. law)? Washington
I'm a buyer in a real estate contract and our original P&S had a form 34 with addendums/amendments which were agreed to by both parties.
4 weeks later I receive a separate form 34 signed by the seller with different conditions.

Question: Does this second Form 34 negate the original or append it?
