My mother-in-law recently suffered her second stroke, which affected her cognitive abilities. A psych evaluation for competency indicated she "is not able to make health and financial decisions" and goes on further to state "the patient is definitely a candidate for guardianship".

My wife is the closest living relative and has assumed the responsibility for care and decision making for her mother. What is the procedure to create a guardianship?
