What forms do you need to file for custody?

what papers do I need when I file for custody at family court. I got a set once and mailed them in w/ photo id, bc's and all but they said they haven't recieved it yet. Been 42 days, so I'm going up there to do it again. I am wondering if I need anything special just to file and I dont know if there is any fees if I go up there either. I dont think so but I'm not sure. Also If I file this so I can move back home, but my spouse has mentioned him going to AA and family counseling if I stay, I dont know if that will even work...but I guess if I decided to try that would be okay even though I filed? Right?
