What is the name of your state?FL
How can I change my daughter's last name from bio-dad's to mine? He lives in another state, I have no current phone number for him, and sometimes receive support-ONLY when taken out of his checks by state.

You have to petition the courts. The problem you run into is the father will have to be served, and most likely consent to the change. If he oposes the name change the most likely it won't be granted.
