Is it possible to have language put into a divorce decree that will enforce an ex-wife to communicate with the children's father about whats going on with them? (leaving the state, educational issues, medical issues, etc)

Yes, it's possible. It's also entirely likely that the mother will ignore said order.

Neither parent needs permission to travel out of state with the child/ren during their parenting time. Would it be nice to know? Probably. But seriously, that one is silly.

Medical records and school records are things both parents can obtain independently of each other.

Finally, if communication is going to be that big of a deal ask the court to order My Family Wizard.
