domestic violence questionnaire

What is the name of your state? california
I recently received a domestic violence questionnaire and have no idea what it is, or its purpose. I do not know the 'custodial person(s)' named on the letter. I do not have any children other than the ones from my current marriage (that I know of). I have been married 3 years, and w/my wife for 7, faithfully. Could someone explain what is the 'meaning' of such a letter? Could it be a mistake? I just want to know what is going on before I call the child support services and try to clarify this situation. I appreciate anyone that has any input. THX!

From the heading , It States If you do not fill it out, Information will be released to other agencies anyways.

You should call the office to find out . Maybe you have the same name as another person,but your Social Security number is Different .
