What forms do I file for arrears if child support case closes. Advice appreciated

What is the name of your state (only U.S. law)? California

Child support service may close my case because they may not be able to locate my ex due to him "MOVING AROUND" and working under the table.
Kids are over 18 and only arrears of about 16k is owed. I want to file a civil suit for arrears because I know how to find him and have him served. DCSS will not do that, they need postal address. I am wondering the correct form # to file. I don't think it will be in the "family law" file, I am thinking a civil suit. THere is alot of forms on the site, not sure where to start and really don't want to pay for a attorney at this point, since I have proof of the arrears and the case has not closed yet with DCSS. I will probably go to the "help center" at the court house but would like to start filling out the forms at home. Any help on the proper form to start on would be appreciated. I am thinking it is the form below.

thanks for your help. What if I filed this? (below) would that just stop by "motion" and then keep case open? The motion was for the arrears, but dcss said they will stay open for at least one more year. Also, I still don't understand why DCSS wants me to fill out a income and expense when my kids are over the age of 20. Its all just arrears.
