Need Example Subpoena for records

My State is Arizona,

Hello gang. I need to Subpoena records from my former employer and can not afford an attorney to do so right now. I call the court and they do not have a set form for me to follow. They said to draw up my own in a legal format and submit it. If it is acceptable they will take it. Does anyone have an expamle of one?
