What is the name of your state? Maine

Is there anywhere online that i can get a form to grant my client the use of a program.. like an end user license agreement?

Is this your program, or a program you purchased or obtained from someone else? If the program is not yours, then you will need to see if your license agreement allows you to sublet the software to someone else.

If it is yours, do a google search for a software license agreement or end user license agreement, you will find some that you can use as templates to create your own license.
