Need Sample Complaint for Property Damage Suit against Co-Op from Water Damage

Hi. Does anyone have any sample complaints where the owner of a co-op apartment brings suit against the co-op association and/or upstairs co-op unit owner for water damge to her unit (ceiling, etc.) from a burst pipe (etc.)? And advice for brining such a suit, or where I could find a sample complaint?
