Addition to Mailing Matrix

What is the name of your state? Michigan

I filed an emergency petition under the old laws (on my own-no lawyer), and I forgot to list a couple of creditors on the mailing matrix. Can I add them now? What type of form should I use?

Also, I have to reaffirm a debt. The only reaffirmation forms that I can find are for the new law. Were there official reaffirmation forms under the old laws?

As far as your matrix you can amend that send in to court your new creditors on a new matrix but only list them. you don't have too fill out a whole new matrix just new ones. But it will cost to amend, Here in washington it cost $26.00 there I don't know. Call your court clerk and ask if there is a form to reafirm.
