is a case dismissed when the judge say's dismissed?

Hi, i am a resident of louisiana. I have been in a chapter 13 since 1998. The judge told us if we surrendered our truck in 1999, we could have our tax refund to replace it, however, we had no refund. In the year 2000 we were converted to a 7, and were told that the offer still stood on the truck, we surrendered the truck in 2000. We were converted back to a 13. since then we asked for a dismissal of the case do to my disability and lack of income. The judge told us the case was dismissed after asking if we could survive without the plan, my attorney assured him with the refund to buy another vehicle we would be fine. only now we are told that the case was not dismissed until signed by the judge which was 4 days later, and our refund was received 1 day prior to that. the judge said it was not done by malice, but she needs to come up with something, so he can release the refund to us, it is frozen at this time. Help Please- i don't understand why the case wasn't dismissed in court that day, Is that really how this works?
