the space provided on this form is very limited. it asks for the creditor's name,address and account #. as i have previously stated all these accounts have since gone to collection. how do i note this correctly. do i put the original credoitor in one box and then the collection agency in the next, simply putting "see box below" in the space provided for the date claim was incurred,etc, on the first line with the original creditor? do i do this in the box that asks amount of claim also?Thanks in advance for your patience... |
