Creditor Questions

What is the name of your state? Kentucky

I am filing Chapter 7 pro se and have 2 questions:

1) When I am listing my creditors, do I need to list my utility companies? I read somewhere that I should.

2) I have a few accounts that have been turned over to collection agencies. How should I list them, under the collection agency or the original account holder or both?

List the OC and then, write sold transfered or, assigned to abc collection agency.
Don't list a utility as a creditor unless, you are trying to discharge an unpaid utility bill.
