Creditors holding unsecured nonpriority claims

I am filling out a chapter 7 voluntary petition. Some of my credit card debt has the original credit card company, the company that took over the account, the collection agency that the account was reffered to, and the legal representative that now holds the claim. All of these refer to the same account, but each has a different account number and amount owed. How should I list these on the form?
