do-it-yourself adversary filing

I want to file an adversary filing (or at least that's what I think it's called) against a creditor that contrary to their own in-house home appraisal amount on my home equity loan falsifies a much larger amount on the contract to justify a larger loan. But against Texas equity lending practices, the loan, unknown to me at the time, zapped 100% of the equity from my home.

My question, does anyone know of a good website to show the format of an adversary filing in bankruptcy court. Several sites would be even more helpful. Your assistance would be appreciated.
