Which forms need to be signed?

What is the name of your state (only U.S. law)? Kentucky

I co-own a home with my ex (unmarried). He has filed chapter 7 bankruptcy, which indicated he wants to "surrender" the property. I am working with my mortgage company under the making home affordable plan. If they decide to modify my loan and I can stay in the house, what steps do I need to take to get my ex's name off the deed and title of the home?
