How do I claim a closed E*Trade Account?

I'm in the process of filing for Chapter 7, and have hit a snag with a closed E*Trade account. I was a joint account member last year, never invested any capital, and the account has been closed since March of last year. Where do I put this information on my petition and how do I indicate that I was only listed on the account for access purposes, even though I never contributed to the capital?
