judgements prior to filing

Prior to filing for protection, in NY, my creditors received judgements and filed leins on my home. I was told that the debts would be forgiven, under a Chapter 13, and I could file a petition to remove the leins. My questions are:
(1)What is the form or designation, I seem to remember something like a Y-512,
(2)Is there a time limitiation to filing these specific forms.
