| First of all, BK forms are FEDERAL, forms are the same no matter what state you're in. Some states require specific state forms.
If you're trying to file pro se, I would strongly suggest you buy the NOLO press book "How to File Chapter 7 Bankruptcy", it gives you everything you need and walks you thru how to fill out the forms.
Payday advance loans - YES include them - they are unsecured debts, nothing priority about them.
As for how far back you go, I suggest that you pull ALL 3 of your credit reports and start going thru them. Any creditor that you haven't paid, went to charge-off or went to collections, or any open account with a balance you need to include. You include ALL collection agencies. If you don't they will come back at you later.
