What is the name of your state? Alabama

I have been contacted by a collector wanting to collect on a medical bill from my daughter from 9 years ago. At that time, I was separated but not divorced from her mother. I don't remember signing any paperwork at the hospital, but the paperwork sent to me by the collector from the hospital shows me to be the guarantor and also has my signature at the top of a page which states it is an authorization for medical treatment, but they cannot produce the page that is preceding my signature. My question is, if my then-wife put my child in the hospital using my insurance, am I responsible for the debt just b/c I'm her father?
