customer approves work then says not what I wanted

i am in the state of florida

I run a embroidery business over the internet. My customers send me jpg files of the designs they want. I create the design with embroidery software, stitch the design and then send then a jpg file of the stitched out design for approval. I never continue if the customer does not like the design. I have a customer that followed this procedure, he approved the design and now that he has received the final product he states he doesn't like it. He has sent another file of the design with a different background and wants me to re do the design and provide him with an additional garment. I created the design from his original artwork, he approved the design. What do I do next. I would also like to include that at first he told me that he had not recived the garments after I mailed them then suddenly they arrived and now he doesn't like them.
