Hi, I am going to pursue arbitration with JAMS in regard to a disputed Directv early termination fee. Here is a link to the form I need to fill out.

I am in NC and I called JAMS, they told me I need to file it with the Orange, CA office.
On page 4 of the arbitration request it asks if this;
COMPLETION OF THIS SECTION IS REQUIRED FOR CLAIMS INITIATED IN CALIFORNIA-
Does this apply to me?
