What is the name of your state? New Mexico

I am having difficulty finding the correct form to use for filing for a Copyright for a design and use of the bag I have produced. Would I use the Visual Arts (VA) form? When I call the copyright office they are not very helpful.

There are many good books that tell you how to copyright items, invest in one, it will be worth it. Most folks you come in contact with at many federal agencies are low paid drones, they are there to shuffle papers, not tell you how to do things. That is why this country has lots of copyright lawyers too. It is the system in place, learn how to work it. Good luck.
